Visit the nearest meeseva centre or visit the concerned municipal or village office to apply.

  1. Application Form
  2. Aadhaar Card/ID Proofs 
  3. Notarized Affidavit
  4. Ration Card/ Any other Residence Proof
  5. Burial Ground Receipt
  6. In case of medico-legal cases
    1. FIR
    2. PM Report
    3. Form 2 given by the concerned police station
  7. Any other supporting documents if any please specify
